Yet Another Clutch Question

So I am completely in love with my car but I haven't been able to completely fix my initial issue.
Here is the problem:

When I bought the car it had significant play in the clutch pedal. I adjusted it and everything seemed to be fine.

However, over several weeks, it seems like the pedal continues to loosen up. I have had to adjust it about 4 times over 3 weeks. I originally thought that I didn't tighten the lock nut enough but upon inspection a few days ago, it seems that the adjustment nut is much further away than where it was originally (now it is much closer to the slave cylinder).

So what is the issue here. I know my clutch is stiff. Is this causing the pedal to fail faster? It looks like most people who have the pedal fail have it fail the other way. In other words, when it fails, it constantly depresses the clutch. In my case the pedal isn't being depressed enough when I push on the pedal.

Thanks in advance.

Check for leaking fluid at the master and the slave. You probably need a new master, as the fluid is passing behind the rubber hat and plastic disc inside. Dont bother with a rebuild.

If so, you need a new pedal assembly.
Well, without looking again I know it has some left and right play. I haven't been able to see any cracks near the mounting points but it is kinda hard to get a good look at those.
