Loose gas cap?

Loose gas cap?
So my 8 likes to throw CELs (specifically the Eccentric shaft position sensor) the sensor has been replaced once and did nothing to remove the CELs so I pretty much ignore it now (dealer seems less than concerned about it).
This week it threw two CELs in the same day (if it's warmed up I just restart it three times and viola, no more CEL) I thought it was a bit odd for her to throw two in the same day, but, I ignored it as usual.
So today I stopped to fill up the gas tank and when I went to remove the gas cap it was loose, really loose, essentially just resting on the fill pipe. Now I always make sure the cap clicks at least three times and I know I put it on tight the last time I filled up.
My questions to you fine folks are:
Has anyone else experienced a gas cap loosening itself?
Is it a common occurrence?
Is there any further action that can prevent this (aside from tightening it upon fill up)?

I've had the loose gascap thing myself, and I think it mostly happens if I am forced into using a full serve station. Some petroleum transfer agents don't properly tighten the cap.

Just came back from dealer. Had a CEL come on last week and stayed on since then. Dealer replaced gas cap said it was faulty and not latching properly. Hopefully this resolved the issue. This was first trip back to dealer since purchase in 6/04

You probably have a faulty gas cap if it keeps popping loose. There are so many aggravating little issues with the 8 and a faulty gas cap is one of them. I had to have mine fixed as well. Its not too big of a deal and its relatively quick fix.

The dealer did something to the gas cap (I don't think it was replaced as the blue part that blocks the fill stem doesn't look new). I think they replaced the plastic/rubber ring that seals the fill stem. It squeaks now when you open it.
Unrelated:
They replaced the E-Brake boot (cracked).
They replaced the "logicon" unit (due to cracked defrost button).
They replaced the battery (due to low voltage - which throws as a CEL code from the eccentric shaft position sensor).
They reflashed to "P".
The car starts much better with the new battery and has finally stopped throwing CELs (I was going to replace it anyway but I'll take a free one for now).
