Clutch pedal came off.
Well, I've decided to start a thread to see exactly how common this problem might be. Within the past two weeks, I and another forum member (TransAmKid) have had our clutch pedals come loose. And by come loose, I mean mine cracked off between the pedal assembly and the firewall and wound up pointed to the right, under the brake pedal.

Early symptoms - While driving, I've often found it hard to shift from 3-4th at high RPMS. It would also regularly grind when shifting from 4th to 5th at any speed. 6th to 5th was also problematic, ginding on occasion. High RPM 1-2 shifts were somewhat problematic (I'd have to wait a moment for the RPMs to drop before engaging second gear). Also, both our clutch pedals squeaked when pressed on.
Late symptom - You push down on the pedal, hear a crack and the pedal faces off to the side. Both my and TAK's wound up to the right, mine was under the brake pedal.
Anyway, I have a feeling there are more than just two of us out there. If you have any of the above symptoms, and you've already checked out the tsb's and known issues, you might want to get under there and check your welds.
The weld that separated on mine was one that connected the clutch pedal assembly to the firewall. There are two there, and the one closer to the passenger side just gave out on me. My car is at the dealer and MNAO is currently giving me the run-around, but I'll get over there either today or tomorrow to take pictures so you can see what to look for.
Charles (a.k.a. Daddy), since you're the one who brought it up in the first place (before my or TAK's pedals actually broke), I was wondering how many times you've come across this issue, or how you knew about it.

Yup, had one customer come in with the same failure at about 60k miles. Spot weld broken at clutch pedal assembly. Only one I've seen. No TSB's to refer to.
Did you charge them for that?
