POSSIBLE CAUSE:
- Rear HO2S heater malfunction
- Connector or terminal malfunction
- Short to power supply in wiring harness between rear HO2S terminal D and PCM terminal 2A
- PCM malfunction
Looks like you need to replace the sensor. $150ish for a new OEM one, though it's just a simple narrowband and a non-critical sensor, so many aftermarket narrowband sensors will work just fine too, which may be cheaper.

Is it difficult to replace? and if you know any DIY would be helpful. would it be worth to buy a new one?

It's easy to replace, you don't have to remove anything else on the car, just unclip the sensor from the harness, then use an O2 sensor socket (or a 22mm wrench if you also remove the shield around the sensor) to unscrew it from the cat. Install the new one by screwing it into the cat, then clipping the sensor it.

It's the one stuck in the side of the cat:

The sensor is non-critical, I ran without one for years. It just monitors the cat efficiency and has a very minor impact on highway cruising mileage (less than 1mpg). You need it to pass OBD2 inspection, if that exists in your state, but otherwise it can be ignored if you want.
