Concern Outline:
On Mazdaspeed6 and RX-8 vehicles, improper installation of the fuel pump mounting rings may lead to cracking of the rings, causing fuel leakage.
RX-8 vehicles ONLY, due to insufficient heat resistance at the top side of the fuel tank, the fuel pump mounting rings may deteriorate due to heat exposure from the engine and exhaust pipe which may also lead to cracking of the rings, causing fuel leakage.
The subject vehicles with such fuel leakage from the fuel pump mounting rings may result in increased risk of fire in the presence of an ignition source near the vehicle.
MAZDA Dealers will install new fuel pump mounting rings on Mazdaspeed6 and RX-8 vehicles.
In addition, dealers will add a thermal insulation pad to the fuel tank on RX-8 vehicles.
There will be no charge for this service to vehicle owners.

Customers who paid to have repairs completed prior to the launch of this campaign will be able to apply on www.mazdareimbursement.com starting November 20, 2021.
In order to be reimbursed for prior repairs, a completed repair order and proof of payment will be required to be uploaded with your completed application.(USA ONLY)

Replace the fuel pump retaining rings with modified ones, tighteningto the correct torque and add thermal insulation pads onto the fuel tank.
UPDATE....
Repair procedure added (.Pdf) for North America (will be the same for rest of world, but repair Part Numbers may vary).
Owners recall letters in NA should have arrived by now or about to in mail box.
NOTE: WHEN YOU TAKE CAR TO MAZDA DEALER FOR RECALL MAKE SURE YOUR GAS TANK IS ON or BELOW 1/4 FULL OF FUEL
AS DEALER MUST DRAIN TANK IF IT HAS MORE THAN 1/4 TANK OF FUEL.
Doing this will make it much easier on Tech doing the work and less time.
Ash - is this a brand new recall to be issued? I just called the local Mazda dealership service department (British Columbia Canada) and was told that is a very old recall. Boom boom is not the same as zoom zoom so I wanted to check. Thanks
NO, it is not an old RECALL and is the FIRST time for Series 2, your BC Dealer is thinking Series 1 which was years ago and are different fuel pumps.
Read what the above says AFTER Nov 20th 2021 owners will get a letter if in USA, CANADA will be after that I would think as they usually are as is rest of the world, why is this so hard to understand?

OK for this S2 ONLY RECALL..so far for NORTH AMERICA.
Both USA and CANADA for their Mazda Dealers use the exact same CAMPAIGN NUMBER 5121I
So I suggest you get your Dealer to actually search as it was notified to Dealers about 5 days ago.
Replacement Parts are still not yet available and I will repeat YOU will get a letter AFTER Nov 20th and not before.
Until that Date and Notification Nothing will happen.
Would this repair involve draining the fuel tank? Also, I wonder if a dealer be willing to perform the recall on a vehicle that does not currently run?
Got my letter yesterday. Currently no engine installed, and once I do have one installed, it's going to be a bit...different. We'll see how they feel about it when the time comes.
My 2010 RX-8 fuel pump seals replaced with a new tool to the dealer. No insulation pad was called out nor was installed by the service technician I discussed the recall closure. So - is the insultation pad not needed or did I get fleeced? What Mazda Said... Parts installed - ST said there was no pad to install. Hmmm...
On the bright side, they let me bring in my car for the recall, left me to wait for over an hour, then came out to tell me they didn't have the correct seal kit. I received a loaner '21 Mazda 3 AWD Hatch premium AT to drive for a week - nice car but doesn't handle near as well as the 8. They did reinstall the S2 lower cover bolt that they lost on the last visit for an oil change. That bolt came all the way from Hiroshima.

I made a point to have an authentic RX-8 bolt as replacement. The ST lost it, never said anything, and when: (1) at the dealership checked that the tires were in fact rotated and (2) I got home I checked for good oil and that the filter was replaced (ceramic marker marked the old filter on). Then I noticed that one bolt was missing...after two calls the ST that did the oil change admitted it "fell behind the tool box" (aren't all tool boxes on wheels these days?) Dealer agreed to replace and I said with the right bolt 'cause it's an engine guard....
I opened a case with MNAO - they said that US dealers are supposed to change both fuel pump rings and all the insulation blanket to prevent Godzilla's heat from causing the rings to deteriorate. Will keep you updated.

The Part Number that Mazda uses for these new o rings is a KIT, the rings and pads are in the bag.(kit).
The Part Number is correct for the REPAIR KIT, just because the description only says Rings is not 100% correct, but this happens. the Kit contains 2 o rings and two heat pads, two retainers and 2 gaskets., total of 8 parts.
My mistake - I was relying on the technician who did the work and the Advisor who both told me there was nothing else but the rings that were installed - I asked about the insultation pad or insultation part and they said there wasn't any. I gues that's my mistake for not removing the rear seats to verify myself (akin to the oil change where I verified they left one bolt off the pan).
Heads up guys on questionable dealer practice. I called my Mazda dealer last week to get the recall scheduled and they told me they would need the car for three days! When I asked why they said they needed to inspect the fuel system so they could order all the parts they needed. I pushed back and noted the recall notice clearly stated what parts needed to be be replaced, to which they replied "its actually a three part recall". When I pressed them even harder on what the extra two parts were they stated "we do not give that information to consumers". I stated "You will not be seeing my car". This is a fairly reputable dealer so I was surprised they acted that way. My guess is that they have done some recalls already and found that other parts like the fuel pump or lines were damaged and had to be be replaced. Even if that were true that is still not part a three step recall. I will be scheduling the recall at a more honest dealer.

They had my car for a week because they didn't order the parts until they had the car in hand. I did drive a new Mazda 3 for that week in the middle of the NE winter. Fun but underpowered car.
Got mine done back in November. Took a day since they pre-ordered the parts. However, the car stunk of gasoline for about a month and they left me an awesome door ding grouping that they wouldn’t take responsibility for.
I've had 2 of my 3 done.. super quick in and out. Currently contacting Mazda Canada to get my 3rd covered. Mazda USA doesn't manage imported cars for recalls.. it's not a global system. Not a complete bummer but was an interesting surprise. FWIW, Mazda Carlsbad can do the recall within a few hours, just come in with less than 1/4 tank gas to make it easy on the tech. I rolled in with 1/8 tank each time.
